Registering with a Doctor
If you wish to register with the practice, please call
into the surgery so that we may give you the relevant
forms to fill out.
All new patients are invited to have
a health check. This health check, along with a
questionnaire, provides us with an opportunity to meet
you and obtain any important background medical history
before your medical records arrive from your former
practice.
Please remember to bring any health related
documents with you when you attend your first
appointment, along with details of any medication that
you may be taking.
