Patients who receive repeat prescriptions for medicines need to have their medication checked regularly. Your repeat prescription tear-off slip shows how many more prescriptions you can have before your next review. Please check this as an overdue review may delay your next prescription request.

Repeat prescriptions can be ordered by ticking the items that you need on your repeat prescription tear-off slip or by writing your name, address and the items on a piece of paper. There is a posting box for repeat prescription requests in each Health Centre, or they can be faxed to 01579 383345 (Callington Health Centre) or 01822 833757 (Gunnislake Health Centre). You can also post requests to us (see top corner for addresses) but please allow sufficient time for delays in the postal service.

Order your prescriptions online:
Please note prescriptions can only be ordered and collected at the surgery where you are registered.

Written requests for repeat prescriptions should be received by the practice at least 2 full working days (longer at Bank Holiday times) before the prescription is needed.

Please use the repeat prescription request form attached to your prescription.

Any amendments to the repeat prescription list can only by made with the doctors permission and will take longer to process.

Prescription payments for dispensed items:
Credit/Debit cards (Callington Health Centre only), cash, or cheques accompanied by cheque guarantee cards, can be accepted as payment. A list of current prescription charges is displayed at each dispensary. Pre-payment application forms are available from each dispensary.
