Change of personal details
If you change your name, address or telephone number,
please notify the reception staff as soon as possible.
Please note: if you are waiting for a hospital
appointment or treatment outside the practice, you will
need also to contact the relevant hospital department to
notify them of any change of personal contact details.
Failure to do so may delay your treatment.
